What to Bring to Appointments

ALL CLAIMS WILL NEED:

  • Original or a certified copy of veteran's military discharge papers (DD-214).
    We can make a certified copy for the claim if you bring us your original
  • Information on all dependents and current spouse:
    Copy of Marriage Certificate.
    Date and place of birth for spouse as well as maiden name.
    Copies of birth certificates for dependent children.
    Social Security Numbers for all dependents including current spouse.
    Death Certificate, if applicable.
    Divorce Decrees for veteran and spouse, if applicable.

Additional information needed for:

DISABILITY CLAIM:

  • Copies of your military medical records if you were injured while in the service.
  • Copies of you current civilian medical records supporting the injury or medical condition you are claiming.
  • If you were treated by a VA Medical Center or other facility, please know the dates and places.

PENSION OR HEALTH CARE ENROLLMENT
(aid and attendance/housebound)

  • Income verification for spouse and veteran for the prior tax year.
  • Other income/investments, including interest and dividend income.
  • Verification of current monthly income.
  • Bank Statements showing current balances.
  • Pharmacy print-out showing monthly prescription costs.
  • Medical evidence supporting need for care and level of disability.
  • Cost of in-home care provider, if paid by veteran and spouse
  • Cost of care facility, i.e. assisted living, foster care, nursing home.
  • A voided check or deposit slip for direct deposit.
