Community Health Nursing Programs
The goal of CCCH Community Nursing is to improve the health of women, infants and children in Clackamas County. Our Community Health Nurses (CHNs) are specially trained Registered Nurses who work with families to provide education to expectant parents and support the growth and development of infants and children. They assist families during pregnancy and after and aid families with high-risk infants and children and children with special healthcare needs.
Community Health Nurses can provide assistance and information and answer questions about:
- Pregnancy, labor or delivery
- Effects of drugs/alcohol/drugs on babies
- Healthy baby care
- Breastfeeding versus bottle feeding
- Healthy nutrition
- Baby’s growth and development
- Keeping baby safe
- Immunizations
- Birth control
- Abuse issues
- Family healthcare
Services are provided in the home and Spanish-speaking staff is available. We do not provide direct patient care to those needing home healthcare. CHN services may be billed to insurance companies and other third party payers. Additional funding sources maybe available to cover the cost for clients without other resources.
